DOS requires copies of all DS-2019 or IAP-66 forms issued to the physician.

Curriculum vitae and state request

The state package includes the physician's curriculum vitae and the state designated official's public-interest waiver letter with the identifying information required by DOS.

Facility and shortage evidence

The package includes a letter from the hiring facility and evidence establishing the applicable HPSA or MUA placement, subject to any qualifying flex arrangement.

Signed employment contract

Current DOS instructions require the signed three-year, 40-hour-per-week contract. If the physician received home-government funding, the additional Conrad-specific No Objection Statement is also required.
