Use the code from the IAP-66

The relevant code should be taken from the applicant's actual Form IAP-66 rather than guessed from a current program description.

DOS specifies a prefix

Current instructions state that the DS-3035 entry should begin with 00. followed by the four-digit subject field code from the IAP-66.

DOS gives 00.2546 as an example

The example illustrates the required format but is not a substitute for the applicant's own IAP-66 code.

Preserve the original program documentation

The IAP-66 copy allows the Waiver Review Division to review the historical program information underlying the DS-3035 entry.
