What Documents Are Needed for a J-1 Advisory Opinion?
DOS asks for the complete exchange visitor record, not only the most recent DS-2019. Missing prior forms or incomplete funding information can prevent an accurate §212(e) analysis.
Every DS-2019 and IAP-66
DOS asks for legible copies of every Form DS-2019 or IAP-66 ever issued to the applicant, including forms from programs for which the applicant believes the requirement has already been fulfilled.
J-1 visa page
The J-1 visa page from the passport should be included. Earlier J visas can also be relevant when the person participated in more than one program.
Program and funding information
The written request should explain program dates and funding sources. Scholarship letters, grant information, sponsor records, or other documents can be useful when government financing is uncertain.
Evidence of fulfillment when relevant
If the applicant believes the two-year obligation was already satisfied, DOS asks for proof of time spent in the qualifying country. The Supplementary Applicant Information Page should also accompany the request.
